State Surveyor Tools & Guidelines
The Survey and Certification Program certifies ESRD facilities for inclusion in the Medicare Program by validating that the care and services of each facility meet specified safety and quality standards, called “Conditions for Coverage.” The Survey and Certification Program provides initial certification of each dialysis facility and ongoing monitoring to ensure that these facilities continue to meet CMS requirements. The Survey and Certification Program recently changed the assessment tools used by state surveyors. The new process, known as the Core Survey Process, is intended to increase the efficiency and effectiveness of ESRD surveys with a focus on contemporary issues in dialysis care including: data use, infection prevention and control, and QAPI. Threads throughout the CORE survey include: culture of safety, safety of dialysis delivery, and the patient voice. Below are links to the CORE Survey Process and important documents that can assist in preparing for a facility survey. One best practice is to keep a binder with the documents needed by surveyors upon arrival (shown on pages 3-6). Keeping this information up to date as needed can assist in preparing your facility for survey and certification.
